Use this search to check if a patient card number has been voided, revoked, or denied.

You can also click on the report to download entire list of voided, revoked, or denied card numbers.

To search, enter the patient card number exactly as it appears on the card, including dashes.

You'll receive one of the following messages:

"The card number you have entered is voided, revoked or denied"
OR
"The card number you entered is not voided, revoked or denied."

**Please note: If the number is not voided, revoked, or denied it does not mean the number is associated with an
active patient registration. **
